Notification of change of address (bank / health fund / employer)

Standard letter to inform a third party (bank, health fund, employer, telecom operator) of your change of address. In addition to the mandatory official declaration to the commune within 8 days.

Good to know: This notification does NOT replace the mandatory declaration to the commune within 8 days (art. 7 of the RD of 16 July 1992). Also think of: bank, health fund, employer, school/nursery, telecom/energy operator, insurers, FPS Finance (via MyMinfin).
